The study reports 21% of companies remain stuck in PoC with no path to live, 27% have only deployed in controlled environments, and only 29% have a quantified business case with measurable ROI. Key failure factors include lack of alignment between IT, business and compliance (49%), data quality issues (32%), and low trust and cultural readiness.
